This is a Free NHS service where our accredited pharmacists have a discussion with you about the medication you take. It is a structured review with patients on multiple medicines, particularly those receiving medicines for long term conditions. During the discussion we attempt to establish a picture of the use of your medicines - both prescribed and non-prescribed. The review will help you understand your therapy and it will identify any problems you are experiencing along with possible solutions. A report of the review will be provided to the patient and to their GP should there be anything for them to consider.

The pharmacy will help support independent living in groups of vulnerable people, or those with special needs, who do not fall within the Disability Discrimination Act 1995 criteria. We can assess your knowledge and use of your medicines and the appropriate level or kind of support, if any, required to help you take the medicines as intended. If support is necessary, the pharmacist will agree with you or your carer the appropriate level or kind of support. We usually do this by keeping in contact with you during the first 4 weeks of your new medicine being prescribed.
